Liberty Sharonville Pediatrics is a Patient Centered Medical Home. As a medical home, we provide comprehensive coordinated care across all elements of healthcare activity including specialty referrals, tracking hospital visits and information on available community support. We are dedicated to providing the best possible care to all of our patients and continue to perform quality improvement activities to enhance that care.
Medical Home Responsibilities:
- Provide office hours, locations and information
- Evidence-based care
- Same day sick visits
- Make office policies available
- Provide after-hours accessibility
- Inquire about and accomodate special needs
- Provide information about available community resources
Patient Responsibilities:
- Provide previous records and immunizations
- Arrive on time for appointment and cancel in a timely manner when needed
- Provide updates on medications, medical history, health status, recent tests, and any visits to other facilities outside of Liberty Sharonville Pediatrics
- Schedule recommended well visits
- Access and share feedback on provided educational and community resources
